Nurse Janice Koh is a Care Manager from Queenstown Polyclinic. She has been working in primary care since 2015 and is currently a member of the evidence-based nursing workgroup in National University Polyclinics (NUP). As a care manager in NUP, her day to day work involves helping patients with chronic disease effectively manage their conditions through education. She also works closely as a team with primary care physicians to review patients with stable chronic diseases. Ms Koh’s research interest lies in chronic disease management and exploring evidence-based practices which are able to help patients improve their health conditions. She is currently the principal investigator for a study related to a nurse-led intervention which aims to optimise diabetes control for patients on insulin.
